Summary:
We are seeking a compassionate and highly skilled Shelter Case Manager to join our team at the Modesto Gospel Mission.
The ideal candidate will have strong case management expertise, the ability to distribute medication, experience with addiction and unhoused populations, and exceptional de-escalation skills.
This role involves managing a caseload of 10 to 15 guests, overseeing up to 50 guests at a time, and requires flexibility in scheduling.
Responsibilities:
1.
Case Management:
Provide comprehensive case management services to a caseload of shelter guests.
Conduct regular assessments and develop individualized plans for guests to address their unique needs and challenges.
2.
Medication Distribution:
Distribute medications to guests so they can administer medications on their own.
Maintain accurate medication records and ensure compliance with relevant health and safety standards.
3.
Guest Management:
Oversee and manage up to 50 shelter guests at a time, ensuring a safe and supportive environment.
Implement and enforce shelter rules and regulations while fostering a respectful and inclusive atmosphere.
4.
Addiction and Co-Occurring Disease:
Demonstrate experience in working with individuals dealing with addiction and co-occurring disease.
Provide support, resources, and referrals to guests addressing substance and mental health concerns.
5.
De-Escalation Skills:
Exhibit strong de-escalation skills to manage challenging situations and diffuse conflicts effectively.
Foster a trauma-informed approach in interactions with guests to create a safe and supportive environment.
6.
Multitasking:
Ability to multitask and prioritize responsibilities efficiently in a fast paced shelter environment.
7.
Flexibility:
Maintain a flexible schedule to accommodate the dynamic needs of the shelter and guests.
Qualifications:
Previous experience in case management, especially with the unhoused population.
Familiarity with addiction and co-occurring disorders.
Excellent communication and interpersonal skills.
Ability to work independently and collaboratively with a team.
Possess a valid driver's license.
Regular church attender and follower of Jesus Christ.
