Clinical Pharmacy Specialist (Ambulatory Care) Bargaining Unit:
Bargaining Unit VA Palo Alto Health Care System (VAPAHCS) has immediate openings for Three (3) Ambulatory Care Clinical Pharmacy Specialists (CPS); one to work from the Stockton Clinic and one to work from the Modesto Clinic. These pharmacists perform clinical functions independently and maintain a scope of practice in specific therapeutic disease states (including, but not limited to:
Diabetes, Hypertension, Hyperlipidemia, Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, first step Pain management, Anticoagulation). These pharmacists are integral members of the Patient Aligned Care Teams (PACT) and work collaboratively with the interdisciplinary team. The duties of this position include, but are not limited to, the following:
Direct Patient Care:
Providing direct patient care for patients assigned to their PACT teams in the ambulatory care setting, which includes:
independent prescribing under a scope of practice, clinical monitoring, ordering labs and coordinating care with other clinicians; performing dashboard surveillance to identify patients in need to clinical intervention for better patient outcomes as measures by HEDIS and SAIL measures; and serving as the pharmacotherapy specialist for the primary care team and providing therapeutic recommendations for clinically challenging patient cases. Safe and Effective Medication Use:
Enforcing VHA treatment and formulary guidelines; providing education to providers to promote appropriate prescribing patterns and adherence to VHA formulary; documenting clinical work; and meeting HEDIS and SAIL metrics. Other:
PGY-1 residents and pharmacy students as assigned; and serving on local and VISN committees as assigned. Basic Requirements:
U.S. citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE:
Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy). TRANSCRIPTS MUST BE SUBMITTED WITH APPLICATION (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). YOU MUST SUBMIT A COPY OF FPGEE AND TOEFL WITH YOUR APPLICATION Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Experience:
Completion of a PGY-1 Pharmacy Residency or at least three years of experience in direct patient care. Grade Determinations:
In addition to the basic requirements for employment stated above, the following criteria must be met when determining the grade of candidates. GS-13 Clinical Pharmacy Specialist (a) Experience or Education. In addition to the basic requirements, candidates must meet one of the following:
1. One year of experience equivalent to the next lower grade level, or 2. Completion of an ACPE-accredited Pharm.D. program. (b) Assignments. This pharmacist is specially trained to work in the ambulatory care clinics of our multi-division health care system. Specific clinics include:
Anticoagulation, Patient Aligned Care Teams (PACT), Telehealth, Mental Health, Pain, and Home Based Primary Care. The incumbent provides coverage of assigned clinics and functions independently, practicing under a scope of practice in specific therapeutic disease states. The incumbent also provides coverage for the pharmacoeconomic team and is assigned to review and adjudicate Prior Authorization Drug Requests (PADRs) on a regular basis. The primary work station will be the Palo Alto campus; however, the incumbent may be scheduled to cover vacancies, vacations, maternity leave, and other extended leave of clinical pharmacy specialists at any of the clinics within the VA Palo Alto Health Care System. This position reports directly to the Associate Chief, Clinical Pharmacy Services with direct oversight for assigned work by the Section Pharmacy Supervisor/Manager. Pharmacists assigned to this position must demonstrate the following knowledge, skills and abilities (KSAs):
Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards pertaining for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. References:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
